Forensic Florida Assertive Community
Treatment (FACT) Team:
The Forensic FACT Team provides highly individualized,
long-term, intensive, assertive community treatment that includes
community-based treatment, rehabilitation, and support services.
Services are provided to adults over the age of 18 who are diagnosed
with a mental illness and have a valid Chapter 916, Florida Statutes
conditional release order. Specifically, the Forensic FACT Team provides
psychopharmacologic treatment; individual therapy; crisis assessment and
intervention; substance abuse treatment; work related services; support
in activities of daily living; social, interpersonal relationship and
leisure time training and support; assistance with securing safe, decent
and affordable living arrangements; case management services; education,
support and consultation to the person’s family and significant others;
competency training; and support and advocacy through the legal system.

Comprehensive Community Services Team (CCST):
Through the Comprehensive Community Services Team (CCST), Suncoast
Center, Inc. provides targeted case
management services, intensive case management services, short-term case
management services, forensic specialist services, conditional release
case management services, housing services, and community integration.
Adult Case Management
Adult Case Management services are designed to provide continuity of
care and improve recovery and quality of life to priority
individuals age 18 or over diagnosed with a severe mental illness.
The focus of services centers on recovery as a continual commitment
to cope with psychiatric disabilities; overcome setbacks; make
productive choices and reclaim one’s quality of life. Primary Case
Management functions are Assessment, Planning, Linking, Monitoring,
and Advocating.
Intensive Case Management
Intensive Case Management (ICM) provides services to individuals 18
years of age and older, who are diagnosed with a severe mental
illness. These individuals have been discharged from a state mental
health hospital, have had several local hospitalizations within a
short period of time, or reside in the community but exhibit
behavior or symptomology that could result in long term
hospitalization if frequent interventions are not provided. Services
are provided weekly and include supporting, advocacy, monitoring,
assessing, planning, linking and brokering on the individual’s
behalf with outside resources such as; residential placement,
employment services and medical and psychiatric appointments.
Short-Term Case Management
Short-term case management is defined as services for less than 30
days. Referrals for short-term case management mostly come from the
medical providers or therapists who work within the Adult Outpatient
Program. Short-term case management services are offered to
individuals who have acute needs, who are at risk of hospitalization
or have been discharged from a hospital. "At risk of
hospitalization" could relate to many things: issues relating to
housing; linkage with basic necessities such as food, or assistance
with getting utilities paid; basically any psychosocial stressor
that may exacerbate a person’s symptoms and result in a
hospitalization. The service expires after 30 days. If at the end of
30 days the person’s needs have not been met, then the case manager
will refer the person to the appropriate service provider.

Vincent House:
Vincent House is a vocational and social recovery program for adults
with mental illnesses. The program is an International Center for
Clubhouse Development (ICCD) certified clubhouse. Suncoast Center
provides this service via a subcontract with Van Gogh’s Palette, Inc.
Persons recovering from mental illnesses are able to relearn vocational
and social skills by participating in a training program known as the
“work-ordered day.” Persons that choose to participate in the clubhouse
are able to perform certain duties in either of the work units; the food
service unit, the business unit or the employment unit as a way to
relearn how to interact with other people and how to complete work
related tasks, thus learning work skills.

Focused Outreach and Intervention Program:
The Focused Outreach and Intervention Program is a jail diversion
and transition program operating at several county locations to provide
services more readily accessible to our clients. The program’s outreach
efforts occur at the Pinellas County Jail and at program offices located
in north and south Pinellas County. The client is linked to continued
treatment services in either north or south county depending on
residency. The program is funded by the Department of Children and
Families.
The program provides ongoing mental health and substance abuse needs
assessment; negotiation and advocacy on behalf of the individual with
the court and other legal systems; supportive case management and
treatment; psychiatric and psychopharmacological treatment with the
provision of psychotropic medications; integrated substance abuse and
mental health individual and psycho-educational group therapy; and
referral services linking individuals to community resources, benefits
and entitlements.
Forensic Intervention:
This program is designed to offer services to individuals who have been
adjudicated incompetent to proceed to trial (ITP), or not guilty by
reason of insanity (NGI), F.S. 916. There are two components to the
program: Competency Restoration and NGI Intervention. These components
are described below:
Competency Restoration
Competency Restoration provides assistance to people who have been
found ITP learn the basics of the legal system in an effort to have
them re-evaluated as competent. The competency program offers
individual or group training with a forensic trainer in the home, in
jail, in the hospital, or at the Suncoast Center main office
building. The competency program utilizes a competency restoration
training curriculum, as well as Team Solutions, an evidence-based
model designed to help individuals better understand how to manage
their illness.
The NGI Intervention
The NGI Intervention piece of the program works with people who have
been found NGI. It provides psycho-educational and stress/time
management training, and reviews the conditions of the conditional
release order. The program offers individual, or group training with
a forensic trainer in the home, in jail, in the hospital, or at the
Suncoast Center main office building. People found NGI are offered
groups and training to support them in their effort to stay in the
community, encourage adherence to the conditions of their release,
and support behaviors that will keep them free of further legal
involvement. NGI Intervention provides similar training to that
received in the state hospital and also incorporates the Team
Solutions model for psycho-education, illness management and
recovery, and stress/anger management.
Each individual’s progress is forwarded to the courts, in an attempt
to reduce the amount of time spent on conditional release.

Adult Outpatient (AOP):
The Adult Outpatient Program provides an array of medical and
non-medical mental health treatment. Medical treatment includes:
psychiatric evaluation; medication management; consultation; laboratory
services related to medication management; a medication line; and open
access groups during office hours in response to urgent concerns
regarding medications. Non-medical treatment services include:
integrated mental health and substance use treatment, biopsychosocial
assessment, treatment planning, individual therapy, group therapy,
crisis intervention, education, family support, psychosocial
rehabilitation services, and referral services. The treatment approaches
are recovery-oriented and focused on individual strengths and resources.

Wellness & Recovery Center (WRC):
The Wellness and Recovery Center is open to anyone recovering from a
mental illness. The Center is designed as a peer-driven, peer-supported
program that promotes recovery and resiliency, alleviates stigma,
increases valuable networks and relationships, and enlists meaningful
use of time. The Center provides an array of services includeing: a warm
line; peer-to-peer support groups; Wellness Recovery Action Planning
groups (WRAP); supportive employment and education services; employment
workshops and fairs; volunteer opportunities and promotion of community
volunteer opportunities; family psycho-educational workshops; wellness
groups; spiritual support/wellness; GED classes; dual recovery support
groups; opportunity to staff and operate the internal household goods
and clothing exchange shop; social event planning; a peer network; and
life coaching opportunities. The Wellness and Recovery Center offers
opportunities to our clients, their family members, and our community.
